If you wish to register with the Meridian Practice you are required to provide documentation from the Home Office UK Border Agency as proof of your entitlement to access NHS services.

This usually takes the form of a letter from the Home Office stating your personal details and a photographic identification card.

Also please bring your medical card if you have one (if you have lost your card our reception staff will be able to help you complete the necessary paperwork to come to a solution).

When you register you will be given a new patient check with practice nurse. Once you have attended this appointment your registration will be complete. For further information about registering at the practice you can call 024 7696 1594 or ask at reception.
